Application Process
1. Request the application form from Shay Hamilton via email (slhamilton4@ua.edu)or phone at 205-348-4666
2. Space in the testing room is limited. Please indicate the 1st and 2nd choice for dates (see dates below)
3. Test times are 9:00 AM and 11:00 AM, please select the time on the application.
4. Email completed applications and payment ($50 per test) no later than 5 days before the Tuesday selected. If the application is received late, the requested day may not be available.
5. Applicants will receive an email notification of the date of the scheduled examination along with a receipt of fees received.
6. Walk-ins will NOT be accepted on the date of the exam.
7. To reschedule or cancel, notify slhamilton4@ua.edu at least 2 days before your examination date to reschedule.

Certification examinations are set and scheduled below. Tests begin at 9:00 AM in Houser Hall, 301 7th Avenue, Suite 2000 on the University of Alabama campus in Tuscaloosa, Alabama on the following dates:
