Course Outline: This four-hour course covers OSHA requirements for maintaining and posting records of occupational injuries and illnesses and reporting specific cases to OSHA. Upon course completion students will have the ability to identify OSHA requirements for recordkeeping, posting and reporting and to complete OSHA Form 300 Log of Work-Related Injuries and Illnesses, OSHA Form 300A Summary of Work-Related Injuries and Illnesses and OSHA Form 301 Injury and Illnesses Incident Report.

Perdido Beach Resort Information: A block of rooms has been reserved at the Perdido Beach Resort for this conference. The available conference room rate is $118 plus 13% tax. The Perdido Beach Resort requires a three night (Sunday–Tuesday) minimum reservation. Rooms for this conference are limited and the host hotel will fill up quickly. Please pay particular attention to any special deposit/cancellation requirements listed below.

Hotel reservations may only be made after you have registered. Once your paid conference registration is confirmed, you will receive an email confirmation with the group number necessary to make your room reservations. Hotel online reservations are not available at this conference. Rooms are limited, so be sure to complete your conference registration and hotel reservation as soon as possible.

The Perdido Beach Resort will allow guests the right to cancel their individual guest room reservation without penalty up to five days prior to arrival. Based on contracted billing arrangements, the Perdido Beach Resort will charge the individual’s credit card for three nights’ room rate for any reservations cancelled within five days of arrival or the guest’s failure to check in. “No show” rooms will be held until 9 am the day following the arrival date on the reservation. Any remaining nights of a “no show” reservation will be cancelled and charged for three nights’ room rate.

If the Perdido Beach Resort fills all room stays, other options are available, however we suggest you call and get on the wait list at the hotel should rooms become available.

Other options include the Phoenix East and East II next door to the Perdido Beach Resort.  We will provide shuttle service.

Many hotels are located close and a parking lot across the street from the Perdido Beach resort is available with shuttle service during the conference.
