Special Notes:
To be awarded a University of Alabama certificate, participants must complete either the OSHA 510, 511, 5410, or 502,503, 5402 or 500, 501,5400, along with additional training noted above. At least four of the courses, including the final two, must be completed through the UA OSHA Training Institute Education Center. Courses taken through the OSHA Training Institute in Chicago, or another OTI-EC, may be used to fulfill two of the course requirements for a UA Safety and Health Management certificate. Any courses not taken at UA must have been completed within three years prior to the student’s official “certification enrollment date” and no later than three years after this date
The Official “Certificate Enrollment Date” is the First Day of the First UA OTI-EC Class the participant wishes to count towards the certificate.
When a participant has completed the entire certificate program, they must fill out an official “Certificate Application Form” online at www.uaosha.ccs.ua.edu. Participants must provide copies of all official OTI Course Certificates to be included in the UA OTI-EC Certificate Program. ALL COURSES MUST BE COMPLETED WITHIN A SIX YEAR PERIOD (a maximum of three years prior and three years after the ‘certificate enrollment date’). Courses cannot be used as credit toward more than one certificate program at The University of Alabama.
